The Consular Section of the U.S. Embassy in Baku accepts applications for new, renewal and replacement passports. Please make an appointment online by clicking on the “Schedule an appointment” link at the bottom of the screen.

Passport services are not available on weekends or on Azerbaijani or U.S. holidays.
Applicants Age 15 or Under Without Both Parents Present
The DS-3053 is the Statement of Consent for Issuing a Passport to a Child and the DS-5525 is the Statement of Special Family Circumstances.
Adult Passport Renewal (DS-82)
Renew an existing passport issued to you when you were aged 16 or older.
Report and Replace Lost/Stolen Passport (DS-64 & DS-11)
Report a passport lost or stolen and replace it.
You can fill out the correct form, or find information about your passport status at https://travel.state.gov.
You must provide one photo on a white background with your passport application. Our photo policies apply to adults and children. We have a few exceptions to our photo policies for infants.
Americans should safeguard passports as they would any other valuable. If your passport is lost or stolen in Azerbaijan, come to the Consular Section to report the loss and apply for a replacement.
